Each participant must have
a signed waiver on file.
Any participant under the age of 18 must have a waiver signed by his/her
legal guardian in the presence of a Doc*36 employee or signature MUST
BE NOTARIZED!
Doc*36 can not accept
any hand written notes or allow a friend or family member to sign
for your child/children. If you are over the age of 18 please be prepared
to show proper ID.
THERE ARE NO EXCEPTIONS!
